Our Responsibilities

We are required by law to maintain the privacy of your protected health information, provide you with this Notice of Privacy Practices, follow the terms of this notice currently in effect, and notify affected individuals following a breach of unsecured protected health information when required by law. We reserve the right to revise this notice at any time. Any revised notice will apply to all protected health information maintained by our practice and will be posted on our website and made available upon request.

If you submit information through our website’s consultation request form, the information you provide is transmitted electronically. Please avoid submitting highly sensitive medical information through online forms. Additional medical history can be discussed during your consultation. While we take reasonable measures to protect electronic communications, no method of transmission over the internet can be guaranteed to be completely secure.

Questions or Complaints

If you believe your privacy rights have been violated, you may contact our office. You also have the right to file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not affect your care or result in retaliation.
